Where can I get a voter registration application?

Pick one up at the Putnam County Board of Elections, 25 Old Route 6, Carmel, NY 10512 or call (845) 808-1300 and ask to have an application sent to you. You may also find registration applications at your Town Hall, local post office, library or motor vehicle office. The application may be downloaded HERE as well.

What to bring

If you've voted in New York before, you don't need to provide ID to vote.


If you're a first-time voter who registered by mail, and didn't provide a copy of your ID with your registration, you may need to show ID to vote. Acceptable forms include:

  • current and valid photo ID
  • copy of a current utility bill
  • bank statement
  • government check
  • paycheck
  • other government document that shows your name and address


If you're a first time voter and unable to provide ID: You will be able to vote an affidavit ballot.

What are the qualifications for voter registration?
  • Be a United States citizen;
  • Be 18 years old by December 31 of the year in which you file this form (note: you must be 18 years old by the date of the general, primary or other election in which you want to vote);
  • A resident of this state and the county, city or village for at least 30 days before the election;
  • Not be in prison for a felony conviction;
  • Not be adjudged mentally incompetent by a court;
  • Not claim the right to simultaneously vote elsewhere.
  • A person who is at least sixteen years of age and who is otherwise qualified to register to vote may preregister to vote and shall be automatically registered upon reaching the age of eligibility as provided by this chapter.

What do I need to bring when I go to vote?

We recommend that you bring your driver’s license to the polling place. Election workers may need to verify identification if a voter’s signature is not on file, if there has been a change of name, or if you are a first time voter who registered by mail.
